Forms

Many Registration & Records-based forms are now available in Workday! 

If you are completing a physical form, please submit the finalized document to the Registration & Records Office by visiting any Aims campus, mailing us, or submitting via email to Records@aims.edu. Please note: all emailed forms must be sent from the student's Aims email address (@aims.edu) in order to be processed.  

General Forms

  • Request to Late Drop or Withdraw from a Course

    The Administrative Registration Change Petition is a form designed for students who have an extenuating circumstance that keeps them from complying with the drop or withdraw deadlines during a term and students wish to change their registration to either a drop or a withdraw.

    Current students may complete this form in Workday by submitting a "Create Request" titled "STUDENT REGISTRATION: Administrative Registration Change Petition".

    If you are no longer an active student and cannot access the form via Workday, please contact records@aims.edu for assistance.

    NOTE: This request requires documentation supporting the extenuating circumstance that the student claims. ARCP forms submitted without supporting documentation will be automatically denied.

  • Request to Late Add a Course

    If you are attempting to add a course to your registration but the deadline to do so has passed, you may fill out the Course Late Add form to request to be added into that class(es).

    Note: In order to be late added into a course, you must have been attending on or prior to the drop deadline for the course in question.

  • Change Your Contact Information

    If you would like to update personal information (legal name, preferred name, address, or other contact info), you may do so through your Workday account. Check under the "Personal Information" section of your home page.

    If you need assistance with this process or you are no longer an active student, please reach out to records@aims.edu for assistance.

  • Change Your Program of Study

    If you would like to update or change your program of study or update your current catalog, you may complete a "create request" in Workday titled "STUDENT PROGRAM OF STUDY: POS Update Request".

    *Note: For questions regarding major selection, please contact your Pathway Advisor at askanadvisor@aims.edu or by calling (970) 339-6443.

  • Students Under the Age of 18

    If you are a student under the age of 18, you must complete the Parent/Legal Guardian Statement for Student Under the Age of 18 form.

    *Note: This form requires the signature of your parent or legal guardian in order to be approved.

  • Applying for Graduation

    All degrees and/or certificates must be declared before applying for graduation. A Graduation Application must be submitted for the degree and/or certificate that you plan to complete that term - otherwise, you cannot potentially be awarded the program. No programs will be retroactively awarded.

    Please ensure you are meeting all graduation requirements, by checking in with your Advisor and reviewing your catalog, before you submit an application.

    NOTE: Make sure you submit a Graduation Application, even if you are not planning to attend the Commencement Ceremony, as that is the only way to have your program awarded.

    If you have any graduation related questions, please email gradapp@aims.edu, via your aims.edu email only, as that email is used for security purposes.

  • Need a Fresh Start?

    Students who have experienced poor academic performance and who have been absent from Aims for a period of two calendar years or longer, may be entitled to a Fresh Start.

    Under this policy, all of a student’s previous grades will be eliminated from the cumulative grade point average calculation. Fresh Start only applies to courses taken at Aims and may only be applied one time. The Fresh Start Program cannot be revoked.

Residency Forms

  • Resolve "Student Action Required" Residency Status

    If your residency status is currently listed as “Student Action Required,” it means that your residency could not be determined from the information listed on your most recent Application for Admission. To remedy that, you may complete a "create request" in Workday titled "STUDENT RESIDENCY: Tuition Classification Addendum" by the first day of the semester in which you intend to register for classes.


    *Note: If you fail to provide this form (in either format) prior to the deadline, your "Student Action Required" residency status will be automatically changed to "Out-of-State" and you will be responsible for paying your student account based on the "Out-of-State" rate until your Colorado residency is determined.

  • Request In-State Residency

    If you are classified as Out-of-State for tuition classification purposes and believe you qualify for In-State tuition, complete the Petition for In-State Tuition Classification and include supporting documentation as directed. This form is not available in Workday.

  • Request In-District Residency

    If your residency is currently listed as In-State for tuition classification purposes and believe you qualify for In-District classification, you may request a change by completing a "create request" in Workday titled "STUDENT RESIDENCY: Request for In-District Tuition Classification request "by the first day of the semester in which you intend to register for classes.

  • COF Affidavit

    If you are a student with undocumented or DACA citizenship and you wish to petition for ASSET tuition rates and classification status, you will need to submit the COF Affidavit form located here along with the "ASSET & High School Graduation In-State Exception Request" form for consideration.

    *ASSET qualification also requires proof of high school graduation.

  • Request for W.U.E. Residency

    If your residency is currently listed as Out-of-State or "Student Action Required" and you are a resident of one of 16 specific states/regions in the western United States, you are eligible to apply to participate in the Western Undergraduate Exchange (WUE) program. WUE students pay reduced tuition rates that are equal to one and one-half times the normal in-state rates instead of the out-of-state rates.

    *Note: W.U.E. states are currently Alaska, Arizona, California, Colorado, Commonwealth of the Northern Mariana Islands, Guam, Hawaii, Idaho, Montana, Nevada, New Mexico, North Dakota, Oregon, South Dakota, Utah, Washington, and Wyoming. 

    Required documents for WUE Classification are:

    A completed "Create Request" in Workday titled STUDENT RESIDENCY: Western Undergraduate Exchange Program Verification of Residency Addendum

  • Active Duty In-State Exception Request Form

    A military member who is on active duty with a permanent change of station (PCS) or is on temporary duty (TDY) in Colorado on the first day of the first class that the student is registered for in an applicable term may be eligible for In-State tuition.

    Once qualified as an In-State student for residency purposes, the member does not lose eligibility if the member retires or separates from the military, nor does the member lose In-State status by reason of presence in any state or country while a member of the armed forces.

    If you believe you qualify for this exception, please complete a physical Active Duty In-State Exception Request Form.

  • Colorado National Guard In-State Exception Form

    If you are a member of the Colorado National Guard (or if you are a dependent of a Colorado National Guard member) and wish to claim Colorado residency despite the length of your physical presence in the state of Colorado, you may choose to submit this form.

  • Request In-State Residency as a Veteran

    All honorably discharged veterans and their dependents who show established domicile in Colorado immediately preceding the start of the semester, regardless of length of time, shall be granted In-State tuition.

    The veteran can also petition for this benefit for their spouse and dependent if the veteran established domicile in Colorado. A dependent is an unmarried undergraduate student and under the age of 23 on or before the first day of class.

    Required documents for application for Colorado GI Promise classification are:

    - A DD-214 showing honorable discharge status and date, AND
    - A completed "Create Request" in Workday titled STUDENT RESIDENCY: US Armed Forces Veteran In-State Tuition Application

    A residency determination cannot be made until all supporting documents have been received. Once a covered individual is determined to have met the qualifications for In-State residence, this person will retain his or her status as long as he or she remains continuously enrolled in the institution.

  • Request In-State Residency under the GI Bill®

    If you are a veteran or dependent of veteran utilizing GI Bill benefits at Aims Community College, you may request residency under Section 702.

    Required documents for application for Section 702 classification are:

    - Certificate of Eligibility (COE) showing eligibility for either Chapter 33 (Post 9/11), Chapter 35 (DEA) or Chapter 30 (GI Bill®), AND
    - A completed "Create Request" in Workday titled STUDENT RESIDENCY: GI Bill Residency Rate In-State Tuition Application

  • Minor Emancipation Claim form

    The process of tuition classification (determining a student’s residency status) is guided by Colorado Statute §23-7-101, et. Seq as laid out by the Colorado Department of Higher Education. Per those statutes, residency can only be based on a “qualified person” – which is defined generally as either the student – in cases where the student is 23 years or older – or a parent or legal guardian – in cases where the student is under 23 years of age.

    If you are under 23 years of age but are not receiving help or support from a parent/legal guardian, you do have the option to petition for "emancipation". If approved, students under 23 years of age would have their residency status determined based on their own information instead of the parent/legal guardian's information.

    If you are interested in this option, please complete the form and email it to Records@aims.edu. All residency deadlines apply.

    Please note that if approved for emancipation via this form, the emancipated status is applicable for purposes of tuition classification only and only here at Aims Community College.

  • High School Graduation In-State Exception Request Form

    Colorado High School Graduate:

    To qualify for an In-State status as a Colorado High School Graduate, a student with any citizenship type other than DACA or undocumented (no lawful status) must meet the following criteria:

    - Attended a public or private high school in Colorado for at least one year immediately preceding the date graduated from a Colorado high school or was physically present in Colorado for at least one year immediately preceding the date the student successfully completed a high school equivalency examination, AND
    - The student has been physically present in Colorado for at least twelve consecutive months prior to enrolling in an institution.

    Students must provide the following documentation to the Registration and Records Office if they would like to qualify for In-State tuition using Colorado High School Grad Exception:

    - An official high school transcript showing the date of graduation from a Colorado institution – OR
    - A copy of the GED diploma earned in the state of Colorado – AND
    - A completed ASSET & High School Graduation In-State Exception Request Form - AND
    - Proof of 12 consecutive months of physical presence in the state of Colorado IF you are applying to Aims 12+ months after graduating High School/being awarded your GED.

  • ASSET In-State Exception Request Form

    ASSET:
    To qualify for ASSET, a student with a citizenship type of either "undocumented" or "Nonresident/Noncitizen" must meet the following criteria:
    - Attended a public or private high school in Colorado for at least one year immediately preceding the date graduated from a Colorado high school or was physically present in Colorado for at least one year immediately preceding the date the student successfully completed a high school equivalency examination, AND
    - The student has been physically present in Colorado for at least twelve consecutive months prior to enrolling in an institution.

    Students must provide the following documentation to the Registration and Records Office if they would like to qualify for In-State tuition using
    ASSET:
    - An official high school transcript showing the date of graduation from a Colorado institution – OR
    - A copy of the GED diploma earned in the state of Colorado – AND
    - A completed COF Affidavit form located here – AND
    - A copy of the ASSET & High School Graduation In-State Exception Request Form - AND
    - Proof of 12 consecutive months of physical presence in the state of Colorado IF you are applying to Aims 12+ months after graduating High School/being awarded your GED.
